Philip is a young man with learning disabilities, epilepsy,cerebral palsy and exhibits behaviour that is on the autistic spectrum which may be considered to be challenging to services. Together provide a specialist outreach service to help Philip's carers facilitate his inclusion and integration into the community.

Albert is a man of 75 years, deemed to have a moderate learning disability and poor physical health. Support provided by Together enables Albert to fulfil his desire to remain in his own home, promoting autonomy and maintenance of life skills whilst participating in many community activities.

Gary is a young man with Down Syndrome who can exhibit aggressive behaviour, particularly towards family and carers. Together provide an outreach service to enable periods of respite for his family during his college holidays.

Joyce is a lady with a dual diagnosis of learning disability and mental health issues. Together's specialist support has averted potential crisis periods for Joyce through early identification and management working in conjunction with community Learning Disability services.

Graham is a man with Prader- Willi syndrome who needs a high level of support to maintain his independence. Together can provide daily domiciliary and outreach support to facilitate Graham's participation in maintaining life skills and accessing the community.

Harry is 87, ten years ago he suffered a stroke. His 82 year old wife has been caring for him, but her deteriorating health is making it difficult for them both. Together are able to assist Harry daily with his personal care making it possible for Harry and his wife to continue living together.

Together can offer domiciliary and outreach services throughout North, South and West Dorset.

Rates to be negotiated on an individual basis depending upon support requirements.
